Modernising Medical Careers (MMC)
Introduction: The Foundation Programme
From August 2005, medical education has been restructured, as part of Modernising Medical Careers (MMC), the modernisation of health care delivery and training in
the National Health Service (NHS). The main thrust of these changes has been the introduction of a two-year Foundation Programme, to bridge the transition
between undergraduate medical school education and postgraduate specialty training.

The foundation programme is intended to be clinically based and patient focused, the object being to deliver high quality, safe medical care. The first year of the
foundation programme, and its standards, have been set out by the General Medical Council in its document, The New Doctor 2005. On satisfactory
completion of the first year (F1), the doctor will obtain full registration to the General Medical Council. F1 is the equivalent of the former Pre-Registration House Officer
Officer (PRHO) year.
The responsibility for the second foundation year (F2) will lie with the Postgraduate Medical Education and Training Board (PMETB). F2 is the equivalent of
the first year of the former Senior House Officer (SHO) grade. At the end of this year, the doctor has to be competent to accept responsibility for patient safety when
practicing 'unsupervised', and must be ready to start the new run-through or specialist training (ST) grades.
The whole foundation programme is intended to be competency based, with increasing levels of complexity in different course settings during the two years. Instead of the
traditional 2x 6 month posts in General Surgery & General Medicine during the old PRHO year, F1 trainees will now usually undertake 3x 4 month rotations. The aim is to
give trainees a broader training experience. For example, more F1 / F2 doctors will work in specialities such as Emergency Medicine and General Practice.

Assessment of trainees
Assessment of a trainees abilities takes place via the completion of regular structured and observed assessment. For the trainee, the reality of assessment means
lots of paperwork, essentially involving the collation of feedback on different aspects of their clinical practise. Most of these assessment tools involve the collection of
feedback from colleagues. Full records of clinical activities, progress and assessment outcomes need to be kept by each doctor in a personal portfolio. Completion
of the Foundation Programme requires completion of the portfolio and the feedback forms. More information on this can be found in the Assessment section
